Fixes:

1) Due to incomplete Microsoft documentation on USB hub driver
(it is a proprietry interface between usbd.sys and usbuhci.sys
drivers) this driver has only limited support for utilities like
usbview.
2) Isochronus transfers untested (in theory they "should" work!).
3) Automated bandwidth allocation not supported.
4) The driver only supports one USB device on either FN0 Port 1 OR 2
(NOT both USB ports at the same time!).
5) USB Hubs are not supported, for this reason, hubs are rejected by
the driver.
6) With rev 5200 chip, occasionally with "DontUseFloudInForBulkPipe = 1"
setting, a 64-byte sized IRP request on a BULK IN pipe may cause IN's
on that pipe to dry up.

1) Fixed DebugFlags initialisation from the registry.
2) Changed Pdo state to "RemovePending" on return from standby so that
IRP's etc are cleanly rejected during whilst the power is restored
to the USB device.
